Please arrive 30 minutes before your appointment to complete a questionnaire and to see the medical assistant. Please bring a copy of your current medication list. 

The following items will be needed for your visit:

• MRI光盘 & Printed Reports, if available.
• Office notes from previous and current physicians, especially neurologists.
• 实验室 & other test results (blood tests, spinal fluid tests, visual testing, etc.)
• If you are unable to keep your appointment for any reason, please notify us by calling 315-464-4243 at least 48 hours prior to your scheduled appointment time, so that we may offer your appointment to another patient.
• If your insurance requires a referral, please contact your Primary Care Provider (PCP) to obtain the required specialist referral, prior to your appointment, in the name of the Doctor you are to see. The referral number must be faxed to us prior to your appointment at (315-464-7328), ATTN: Neurodegenerative Disorders referral
• Particular follow-up times may be requested through our patient portal.  Notes, lab, and imaging tests may also be reviewed.  Patients may register for this portal at the end of their first visit, a sign in code will be generated on the after-visit summary to log into Mychart.

What to Expect at 你约会的时间 

• What happens in a new patient appointment and what can I expect?
  A new patient’s initial clinic appointment will take about 60-90 minutes depending on a disease category and will consist of reviewing a patient’s medical, neurologic and neuropsychiatric histories, 临床检查, discussion and counseling of the patient’s and family members’ concerns, and development of a follow-up treatment plan. Our clinicians may also order more in-depth tests for the patient that includes blood draws, 神经成像扫描 (e.g.、PET、MRI等.), neuropsychological tests or lumbar puncture procedures.
• Will I have testing the same day as my appointment?
  通常, 神经影像学, neuropsychological tests and lumbar puncture procedures are scheduled on separate dates. You may have blood drawn the same day.
• Why does someone have to accompany me and why can’t I come alone?
  Since many our patients have memory problems or other cognitive concerns, we require our patients to come to their clinic appointments with a close family member or an individual who can provide reliable information about their medical condition.
• How often will I have appointments?
  After the first appointment, our clinicians may order additional tests and a follow-up visit (known as a Result Discussion and Management plan visit) to review test results will be scheduled. After the Result Discussion and Management Plan visit, most patients will be seen about every 6 months for follow-up visits for cognitive patients, 然而, movement disorders patient may be seen every 3-6 months
• Should I bring anything?

• How long are the appointments?
